When Submitting the form or Application Below we would like for you to include the following
information about the home you will be providing for the puppy.
1. Which Puppy are you considering?
2. Have you ever owned a Yorkie Before?...How many current Pets do you own, what type and
are they altered ( spay/nuetered) ?
3. How many in your household?
4. Do you work outside the home, if so how many hours will the puppy be left alone each day?
5. What knowledge do you have of Toy Breeds?
6. Have you had to re-home any Pets in the last 5 years, if so why?
7. Were you wanting just for a Pet or are you interested in Show or Breeding? ( Our Yorkies are
placed as Companion Pets only).
8.Are you willing to give your Veterinarian Reference?
We are very careful with the placement of our puppies. We believe there is a right match with
every puppy and family. This is not a task I take lightly when placing one of my babies. Please
list as much info as possible and feel free to call me to seriously be considered. Thank you for
considering one of my puppies~
